Summary:
The Community Health Worker (CHW) provides coordination of care activities and facilitates access to community resources in an effort to support population health in the community and the primary care setting.
The CHW#s role is to improve health and wellness by promoting healthy lifestyles, building community partnerships, and providing care coordination for vulnerable patients.
# Through an integrated approach, this role conducts home and office visits, telephonic follow-up for patients and their families to navigate and access community-based services and resources.
# This position provides outreach, social support, and care navigation while working collaboratively within the department, primary care practice managers, physicians, and care team.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES:
Education # Training:
High School diploma or equivalent is required.
# Certified Nursing Assistants, Certified Medical Assistant preferred.
BLS Certification is required.

Experience:
Must have at least one (1) year experience working in a health care environment, medical practice, or community-based setting (such as social services, supportive services, personal care) or related duties working with adult/senior populations.
Skills # Abilities:
# Demonstrate skill in a) the ability to be self-motivated and function on an interdisciplinary team; b) have the ability and willingness to provide direct support, encouragement and motivation to clients;# c) demonstrate solid written, interpersonal, and oral communication skills;# d) prioritize work assignments and manage time effectively to complete duties; e) work effectively and calmly in a fast paced environment; f) have proficient understanding of community resources; g) effective use of EMR, email, word processing, presentation and spreadsheets; h) demonstrate an understanding and sensitivity to serving a culturally diverse population while respecting patient confidentially; and i) must have reliable transportation.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
