Summary:
# Manages the safe and effective use of anticoagulation therapy for patients of University of Maryland Upper Chesapeake Health System.
# Provides and supervises daily patient-care activities of:
providing patient education, consultation, therapeutic drug monitoring for efficiency, safety, evaluating drug interaction potential, making appropriate dosage adjustments and effectively communicating to the patient and/or care-provider, physician and the healthcare team.
# Coordinates the daily operations, planning, collecting data and identifying performance improvement opportunities.
# Supervises the work of technical and clerical support personnel in the clinic setting.
Education # Training:
##Bachelors of Science in Pharmacy from an accredited college of pharmacy required.
Doctorate of Pharmacy, Hospital Residency, managing anticoagulation therapy in patients, or similar experience desirable.
# Licensed, or eligible, to practice pharmacy in the State of Maryland.
# Must obtain certification in managing anticoagulation therapy within 1 year from date-of-hire.
Work Orientation #
Experience:
##Three years Pharmacist experience preferred.
Previous experience in anticoagulation therapy management and previous supervisory experience highly preferred.
# The incumbent may require up to three months of detailed job orientation, training and self-study to gain specific capabilities to perform essential job functions required of hospital /ambulatory care pharmacists.

##Excellent written and oral communication skills.
# Interpersonal skills necessary to communicate effectively with patients, physicians, nurses and health-care team members about prescription use and obtaining or exchanging relevant patient information.
# Ability to communicate with patients of various ages.

Estimated Salary: $20 to $28 per hour based on qualifications.
