Summary: Healthy Harford, Inc., founded in 1993, is the Healthy Communities Initiative of Harford County and doing business as Healthy Cecil. The mission of Healthy Harford/Healthy Cecil is to inspire and empower healthy people, healthy families and healthy communities in mind, body and spirit. The focus is to improve health and wellness in Harford and Cecil Counties by promoting healthy lifestyles, building community partnerships and providing care coordination.
The Community Health Worker (CHW) is a member of the Healthy Harford/Healthy Cecil Community-Based Care Team. Through an integrated approach, the CHW will be responsible for conducting home visits, and helping clients and their families to navigate and access community-based services and resources, in order to adopt healthy behaviors. The CHW services will include outreach, social support, care transition and community service linkages.
Education & Training: High School diploma or equivalent. Certified Nursing Assistants, Certified Medical Assistant preferred. BLS required.
Work Orientation &
Experience: 1-2 years of experience working in a health care environment, medical practice or community-based setting such as social services, supportive services, personal care or related duties working with the adult or senior populations. Basic computer skills required. Successful completion of a Community Health Worker training program and Mental Health First Aid Certification will be required.
Knowledge, Skills & Abilities: a) demonstrate the ability to be self-motivated and function on an interdisciplinary team; b) have the ability and willingness to provide direct support, encouragement and motivation to clients; c) demonstrate solid written, interpersonal, and oral communication skills; d) prioritize work assignments and manage time effectively to complete duties; e) work effectively and calmly in a fast paced environment; f) have proficient understanding of community resources; g) be available to adapt hours to meet the needs of all clients; h) demonstrate an understanding and sensitivity to serving a culturally diverse population while respecting patient confidentially; and i) must have reliable transportation.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
